Health and Wellbeing Coach

Health & Wellbeing Services

Health and Wellbeing Coaches engage with local people who require practical support to improve their health and wellbeing and change their behaviour to achieve their own choices and goas. A free & confidential service offering support on a one to one basis and in a group setting. Coaches will support you to develop a Personal Health Plan, which will help you to achieve your goal. The goals can focus on physical & mental well-being, offering personalised advice on healthy eating, weight loss, increase in physical activity, stress management, and self-care, social isolation, low mood and more.


Health & Wellbeing Coaches also support people living with long term conditions (chronic pain & diabetes type 2).


What We Offer:

  • Support, encouragement and motivation to develop your own health goals

  • 7–8 personalised support sessions on a one-to-one basis

  • Identify groups or activities in your area

  • Guidance on healthy eating, weight loss, physical activity, stress management, self-care and mental wellbeing

  • Referrals and signposting to appropriate services and activities

  • Friendly, supportive coaches who will encourage and guide you toward a healthier lifestyle


Who Is It For

This service is designed for individuals of any gender who feel ready to take meaningful steps toward improving their health and wellbeing.


How to Join

A Referral is made to ShipShape Community Hub by either your GP Practice, a professional service, or self-referral.
